The Healing Tradition of the Island of the Gods

Balinese healing is an ancient holistic healing system that has been practiced on the island of Bali for over a millennium. It is a synthesis of Hindu philosophy, animist traditions, herbal medicine, energy work, and spiritual purification—a complete system designed to restore balance between the physical body, the spirit, and the cosmos.

The foundational philosophy of Balinese healing is that health is a state of balance—balance between the elements, between the visible and invisible worlds, between the forces of creation and destruction. This balance is understood through the lens of the Three Realms, the Tri Hita Karana, and the Rwa Bhineda—the duality that governs all existence.

The Balian: Healer, Priest, and Psychopomp

The practitioners of Balinese healing are known as balian (in Balinese) or dukun (in Indonesian). The balian is not merely a healer—they are a spiritual guide, a priest, a psychopomp, and a guardian of ancient knowledge. They serve as intermediaries between the human world and the spirit world, diagnosing illness not only through physical symptoms but through spiritual and metaphysical causes.

Type of BalianRole
Balian UsadaHerbalist and traditional physician—specializes in plant-based remedies and massage
Balian TaksuSpiritual healer—works with energy, mantras, and spiritual purification
Balian TenungDiviner—diagnoses through trance and spiritual vision
Balian SihPsychic healer—communicates with spirits and ancestors
Balian KekawinMantra healer—uses sacred texts and incantations

The balian undergoes years of training, often inheriting their knowledge through oral tradition and direct apprenticeship. Their power is believed to come from taksu—a divine spiritual energy that flows through them, enabling them to channel healing forces from the Upper Realm.

Core Healing Practices

1. Usada: The Herbal Tradition

Usada is the name for Balinese herbal medicine—a vast pharmacopoeia of plant-based remedies that treats everything from common fevers to chronic conditions. The knowledge of usada is recorded in ancient palm-leaf manuscripts (lontar), written in the Kawi and Balinese scripts.

RemedyPurpose
JamuA traditional herbal drink—turmeric, ginger, tamarind, and other medicinal plants
BorehA warming herbal paste applied to the body—used for muscle pain and stiffness
Minyak KelapaCoconut oil infused with herbs—used for massage and topical healing
LulurHerbal body scrub—used for purification and skin health

The usada tradition is closely tied to Jamu, the broader Indonesian herbal medicine system.

2. Energy Work and Spiritual Healing

Balinese healing recognizes that illness often originates in the spiritual realm before manifesting in the physical body. Healers work with the body’s energy centers (cakra) and the flow of spiritual energy (mana) to restore balance.

PracticeDescription
Pijat BaliTraditional Balinese massage—combines acupressure, stretching, and energy work
Cakra BalancingRestoring balance to the body’s energy centers
Mantra HealingReciting sacred verses (mantra) to invoke divine healing power
Spiritual PurificationUsing [[bali-agama-tirtha

3. The Melukat Purification Ritual

Melukat is the sacred purification ritual that uses holy water (tirtha) to cleanse the body, mind, and soul of negative energies. It is considered one of the most powerful healing practices in Balinese tradition—a remedy that addresses the root cause of illness rather than merely its symptoms.

4. Medicinal Offerings

Balinese healing also involves offerings (banten) prepared for specific purposes. These offerings are presented to the gods, ancestors, and spirits of the Fourth Realm, requesting their assistance in the healing process. The offerings are believed to carry prayers and intentions to the Upper and Lower Realms, creating a bridge between the patient and the divine.

Healing and Cosmology

In Balinese philosophy, healing is understood through the lens of balance, duality, and cosmic order. Illness is seen as a disruption of harmony:

ElementCause of Imbalance
Tri Hita KaranaDisharmony with God, with others, or with nature
Rwa BhinedaAn excess of one force over its opposite
Three RealmsA disturbance in the connection between the human realm and the spirit realm

The balian restores balance by addressing the patient’s physical, emotional, and spiritual needs through a combination of herbs, massage, energy work, and spiritual purification.

Story Hook: The Healer Who Heard the Spirits

A balian usada in a remote mountain village inherits a sheaf of ancient palm-leaf manuscripts (lontar) from her grandmother. She has been trained in the traditional remedies, but the manuscripts contain something more—a section she was never taught, written in a script she cannot read.

With the help of an elder priest, she deciphers the text. It contains a description of a healing ritual that has not been performed in seven generations—a ritual that invokes the power of the Fourth Realm directly. The ritual requires a healer to travel to the boundary between realms, speak with the spirits of the hidden layer, and request their assistance in healing the community.

But the ritual carries a price: the healer must offer a part of herself in exchange—a memory, a sense, a piece of her soul.

She must decide: is the cure worth the cost? And if she crosses the boundary, will she be able to return?

“I have healed the sick, soothed the dying, and guided the souls of the departed. I have walked the path of the balian for forty years. But this ritual—this one is not a healing. It is a reckoning. And I am not sure I am ready.” — A Balinese balian usada reading her grandmother’s lontar

Visual Imagery

Balinese healing is often depicted in art and photography as:

  • A balian dressed in white, holding a bowl of holy water and a bundle of herbs
  • Palm-leaf manuscripts (lontar) with ancient script, resting on a wooden stand
  • The melukat ritual at Tirta Empul—worshipers kneeling under the spouts of holy water
  • A traditional Balinese massage—the healer’s hands pressing and stretching the body
  • Offerings (canang sari) arranged on a patient’s bed, accompanying the healing process
